Faculty Annual Assessment Form
Please complete and submit the Faculty Annual Assessment form to the Department Chair by April 15th each year. If additional space is needed please include that information in the email when sending the survey or add an additional page with the hard copy.
After completing the form submit it by saving and emailing it to the Department Chair; or printing out the form and submitting the hard copy.
